Employee Leave Form
This form is for all Employees (Crew & Managers) to request leave (annual leave & sick leave etc).
If you have enough sick or annual leave in your balance to cover your time off, you need to apply for that leave.
LWOP (Leave without Pay)/'I don't want to be paid any leave' - should only be used when you do not have enough leave to cover your time off.
Eg, 3 hour sick leave in balance, but 8 hours shift was sick. You can either use the 3 hours and have LWOP for the remainder, or choose LWOP.
Casual crew can also use this form to request large periods of time off (7+ days).
You can communicate days you want off with the Roster Manager by using the 'NA' diary in store.
Please ask a Shift Manager or Crew Trainer if you are not sure where it is, or how to use it.

Annual leave provides employees with the opportunity to have a break during the working year and should be taken as it becomes due, at a mutually agreed time, or as directed. An employee's request to take annual leave will not be unreasonably refused. Occasionally there may be exceptional work circumstances which prevent an employee from taking their leave; however it is the responsibility of managers to balance the needs of the department alongside the entitlement to take leave.
Likewise, an employer can require an employee to take a period of annual leave if the requirement is reasonable. A direction to take annual leave would be reasonable where an employee has accrued an excessive amount of annual leave; or the business is being shutdown for a period.
Permanent employees are entitled to 4 weeks annual leave per year worked; pro-rate for part time employees and those with less than 12 months continuous service. Annual leave does not apply to Casuals, as they are compensated for this through their hourly rate. Continuous seven day shift workers are entitled to additional benefits under legislation.
